Hormone Replacement Therapy

For patients seeking hormone therapy, please be advised that our physician requires a letter from your counselor stating that you have completed the required counseling to educate & prepare yourself for the physiological changes that will occur during hormone therapy.

Mental Health & Counseling Referrals

We have a counseling referral list for patients seeking mental health counseling or referral letters to begin hormone replacement therapy.

The goal of AWC Trans* Health Initiative is to provide excellent health care for all transgender and non-binary clients by:

  • Transforming the way transgender people receive care and help to eliminate barriers to achieving maximum health and well being
  • Provide a safe, comfortable & respectful clinic for all transgender and gender non-conforming people to receive care
  • Collaborate with other healthcare organizations serving the transgender community

To schedule an appointment, call any time during our office hours. If you are using insurance, please have your insurance information available. Fees for office visits are based on your insurance eligibility or the AWC fee structure. These options can be discussed when you make your appointment. Applicable records can be faxed to AWC or brought in on the day of your appointment.

Hormone Replacement Therapy

  • First Medical Appointment
    • Medical History: You will be asked to fill out paperwork pertaining to your medical and family health history
    • Physical Exam (If you have had a physical exam in the past year, you can either bring those records to your appointment or have them faxed to AWC.)
    • Lab Work
    • You’ll have a chance to meet with our Medical Director to discuss the medical considerations for beginning and/or continuing your transition hormonally
  • Second Medical Appointment (Must have referral letter from counselor/therapist)
    • Review lab work with the Medical Director
    • Discuss next steps in your care
  • On-going Care
    • Once prescribed hormones, you should expect to return to AWC within three months
    • Blood work will be done to monitor hormone levels
    • If you are using injectable hormones, we will offer self-injection lessons with our medical support staff

Mental Health & Counseling Referrals

  • AWC does require a referral letter from a counselor/therapist in order to begin hormone therapy.
  • A letter is not required of patients who have already begun hormone therapy and are transferring to AWC.
  • For a list of mental health and counseling referrals, please call AWC at 484-821-0821.
  • AWC does have counseling referrals who offer Skype®

Referral letters from your counselor/therapist can be faxed to AWC or brought in on the day of your appointment.
